We ask that you arrive about 10 minutes early so that you have enough time to fill out our New Patient Form.*
Initial Examination
We will begin by reviewing your medical and dental history before completing a comprehensive exam of your mouth and oral structures.
Your exam will include a thorough assessment of your gums, teeth, jaw and bite as well as an oral cancer screening and digital X-rays.
Treatment Plan
Depending on your medical history and the results of your examination, we will develop a long-term dental treatment plan for you.
We will also take time to answer any questions you may have about your oral health and treatment options.
**Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.
